National Healthcare Decisions day is on Tuesday, April 16, 2019. The day is set aside to educate and empower the public and health-care providers to take part in important advance care planning, making sure that information, opportunities and access to documents are available to all adults.

On Monday, April 22 at 7 p.m., St. Rita’s Hospice will present information on Ohio’s Advance Directives in the Gaspar Room, at St. Charles Center in Carthagena, Ohio. This is a follow-up to a presentation by Fr. Tom Welk, C.PP.S., “Having the Conversation,” that was held in December.

At 7:45 p.m., staff will be available to assist those who wish to complete the documents. Participants are invited to bring along the names, addresses and telephone numbers of those whom they would like to include in any documents. Advance directive forms will be available.
